      <title>BIOELECTRICAL IMPEDANCE TECHNOLOGY FOR EVALUATING HUMAN BODY COMPOSITION PARAMETERS: “AN ADVANCED DIAGNOSTIC TECHNOLOGY FOR BODY COMPOSITION AND PHYSICAL FITNESS ANALYSIS”</title>

ABSTRACT:
Enormous use of sophisticated technology in daily living has resulted in lifestyle disorders in form of obesity, hypertension, cardiac disorders and musculoskeletal problems in a community as a whole. This significant rise in physically unfit community prompted us to find a most cost-effective, simple, and reliable mode of physical fitness evaluation in form of Bioelectrical Impedance technology. An observational randomized experimental study was conducted at Multispecialty Department of Physiotherapy, Bombay Hospital-Indore-India. Tanita BC-418 working on the principle of Bioelectrical Impedance Technology was used for Body composition Analysis of 100 male and females with age group 40-50 years. Conclusion: BIA has proved to be an advanced diagnostic technology for evaluating human body fitness in today’s fast and rapidly advancing life style. The Body Composition Parameters important in evaluating physical fitness such as Body fat%, Fat Free Mass, Total Body Water%, BMR, segmental fat distribution can be easily assessed with this technique. Multi-frequency segmental Bioelectrical Impedance analysis identifies the Differences in segmental body fat% distribution and these differences signify the presence of regional pain, having musculoskeletal origin.

      <title>DIAGNOSTIC ACCURACY OF PROVOCATIVE TESTS IN LATERAL EPICONDYLITIS</title>

ABSTRACT:
The aim of the present study was to analyze the diagnostic accuracy of the commonly used provocative tests in the diagnosis of lateral epicondylitis (LE). Cozen’s test, Mills test and Maudsley test are most widely used. Till date no studies have been reported on the diagnostic accuracy of these tests. Musculoskeletal ultrasonography serves as a gold standard tool in the diagnosis of LE. Thirty subjects participated in the study. Baseline measurements of pain severity, elbow joint mobility, hand grip strength and three provocative tests were recorded by the principal investigator. A second investigator accompanied the subjects for musculoskeletal ultrasonography who was blinded of the test results. The thickness of common extensor tendon, echo texture and lateral epicondyle bony contour was measured. The test results of the three provocative tests with ultrasonographic findings were analyzed. The sensitivity for Cozen’s test, Maudsley test and Mills test was found to be 84%, 88% and 53% respectively. The specificity for Cozen’s Maudsley and Mills test was found to be 0%, 0% and 100% respectively. Mills test showed significant area under receiver operator curve (ROC) i.e. (0.769), which explains that the test has good diagnostic accuracy. This validation study, concludes that Mills test has an excellent diagnostic value for ruling in LE.

ABSTRACT:
The present review study is carried out in order to study various practices in physiotherapy across different places. Physiotherapy is the health care profession primarily concerned with the remediation of impairments and disabilities and the promotion of mobility, functional ability, quality of life and movement potential through examination, evaluation, diagnosis and physical intervention. Physical therapy is a professional career which has many specialties including sports, neurology, woundcare, cardiopulmonary, geriatrics, orthopedic and pediatrics. The study concluded different practices in physiotherapy treatment across various places. Some authors mentioned very clearly the under utilization of physiotherapists in providing rehabilitation services and were of the strong opinion that the physiotherapists should be actively involved as they could play an important role in the implementation of a cost-effective rehabilitation service paradigm. One author examined the origin of physiotherapy and particularly its relation with the nursing. Some authors studied the trends and the desirability of specialization in physiotherapy practice, opportunities for individual practitioners to gain recognition as specialists, and the Scope for achieving greater harmonization of specialist practice and recognition across the EU. Few authors were of the view that as a profession integral to health promotion, prevention, acute care and rehabilitation, physiotherapy plays an essential role in the health care system. Some researchers pointed out the lack of evidence of use of tilt tables by physiotherapists in Intensive Care Units (ICUs) in Australian Hospitals. A group of researchers also threw a light on the practice and research of complementary and alternative medicine (CAM) in Canada and few discussed how physiotherapists might respond to the challenges of health care reform which took place in New Zealand some eight to ten years back. A group of authors brought to the limelight a very important observation which stated that the Physiotherapists are known to be prone to Work- related musculoskeletal disorders (WRMDs) but its prevalence among physiotherapists in Nigeria has not been reported.

      <title>EFFECT OF ICE BAG, DYNAMIC STRETCHING AND COMBINED TREATMENTS ON THE PREVENTION AND TREATMENT OF DELAY ONSET MUSCLE SORENESS </title>

ABSTRACT:
Objective: To investigate the effects of ice bag, dynamic stretching, combined ice and dynamic stretching, and control (non-treated) on the prevention and treatment of delayed onset muscle soreness (DOMS) in biceps muscle.
Subjects: Fifty-five participants (aged 18 to 25 years) were engaged in this study and randomly assigned into four groups (control group (non-treated) (CG), n = 13; ice bag, n = 14; dynamic stretching, n = 14; and combined treatment, n = 14).
Method: Before inducing DOMS, the range of motion (ROM) and maximum voluntary contraction (MVC) were measured. The dynamic stretching was performed before inducing DOMS. Subjects performed biceps eccentric exercise at 110% of the predicted one-repetition maximum (1-RM), for each subject, to induce muscle soreness. Pain, ROM and MVC were assessed at 0, 24, 48, 72, and 96 hours after induction of DOMS.
Results: These non-significant results for mode of treatment and time interaction showed that combined treatment, ice bag, or dynamic stretching alone is not effective at significantly reducing the symptoms of DOMS.
Conclusion: These results are non-significant, the pattern of the data showed that the combined treatment may be contraindicated in the prevention of DOMS and ice bag or dynamic stretching might be the best choice of treatment. Further investigation is strongly recommended.

ABSTRACT:
Background: The ability to jump plays an effective and important role in volleyball, Because jump skills are greatly complicated that it is nearly the outcome of vertical force and horizontal speed besides harmony and synchronization of the work of arms and feet. There is also total harmony related to the skill and plan achievement during attack and block.
The purpose of this study was to examine the Effect of Plyometric VS. Pilates Exercises on the Muscular Ability and Components of Jumping in Volleyball Players.
Study Design: Experimental design.
Methods: 30 subjects were selected randomly from the population using simple random sampling procedure and were divided into two equal groups.  Group A was given plyomeric training and Group B was given Pilate training.  Outcome measures were taken before and after the Program Schedule of 3 Sessions alternately in 1 week for 6week.
Outcome measures: Vertical Jump height, Block jump, and the attack jump, Agility T test,
Results: In Group-A (plyometric ) and Group-B (Pilate), all data was expressed as mean ± , SD and was statistically analyzed using paired ‘t’ test and independent ‘t’ test to determine the statistical difference among the parameters at 0.5% level of significance. Statistical data of agility t test, vertical jump height, the Block jump, and the attack jump in volley ball players showed that, there was no significantly difference between groups. And both were effective with p<0.05; i.e 95% of significance.
Conclusion: In this study, we concluded that both groups (A & B) were effective in agility t test, improving vertical jump height, the Block jump, and the attack jump in volley ball players. But we recommend use of plyomertic training in volleyball players.

ABSTRACT:
Background: Cervical radiculopathy is a dysfunction of nerve root of the cervical spine where C6& C7 nerve roots are the most commonly affected. It encompasses important symptoms other than pain, such as paresthesia, numbness and muscle weakness in dermatomal or myotomal distribution of an affected nerve root. A multitude of physical therapy interventions have been proposed to be effective in the management of cervical radiculopathy, including mechanical cervical traction, manipulation, therapeutic exercises and TENS. Studies to find out the effectiveness of TENS versus Intermittent Cervical Traction among patients with Cervical Radiculopathy are sparse. 
Hence the present study was undertaken to find out and compare effectiveness of TENS versus Intermittent Cervical Traction a newer technique towards betterment in treatment of cervical radiculopathy patients.
Methodology: 30 patients from Baroda association for the blind (Lions club of Baroda), Subhanpura & Sushrut Physiotherapy Clinic, Akota were chosen based on the inclusion and exclusion criteria. Group A comprised of 15 people with cervical radiculopathy were given TENS with Isometric neck exercises and active neck movements. Group B comprised of 15 people with cervical radiculopathy were given Intermittent Cervical Traction with Isometric neck exercise and active neck movements. VAS Scale & Neck Disability Index (NDI) were used as outcome measures pre & post treatment.
Results: The pre test evaluation showed that, there is no significant difference (P> 0.05) between the two groups for all the variables measured. The post-test evaluation of both groups showed a very high significance (P< 0.05) within the group for all the outcome measurements. A post-test comparison of measured variables, between the groups showed that the Group A demonstrated a statistically significant (P< 0.05) reduction in pain and Neck Disability Index.
Conclusion: From the above study concluded  that TENS was more effective in the management of cervical radiculopathy along with isometricneck exercise, in reducing both neck & arm pain, neck disability & in improving activities of daily living.

ABSTRACT:
Introduction: Stroke related hemiparesis also exhibit asymmetry in standing posture and walking. Chronic stroke survivors represent with asymmetric gait pattern due to altered weight distribution. Altered weight distribution of lower limbs that is less weight is taken on affected side, fewer excursions on the weaker side, is seen during static and dynamic balance as well as external perturbation.
Objective: To evaluate the effect of Compelled Body Weight Shift Therapy on weight bearing symmetry and balance in post stroke patients.
Methods: Total 22 participants were included in the study. All patients received 10mm shoe insole which is to be used on their unaffected side and along with this conventional rehabilitation program were given for 1 hour per day for 2 weeks.
Results: There was significant difference in pre-post weight bearing distribution (p<0.001) and balance (p<0.001). There was also significant difference in all the component of Berg Balance Scale (p<0.001). 
Conclusion: Compelled Body Weight Shift Therapy can be easily administrated in the daily rehabilitation protocol, while treating stroke patients with balance problems due to asymmetrical weight bearing. With more symmetrical weight bearing, balance can be improved for better ambulation and reduced risk of falls.

ABSTRACT:
Background: In the prevailing era of adult hypertension, limited data are available regarding the profile of childhood obesity & hypertension from India. We examined & studied the prevalence of childhood obesity & hypertension in a representative sample of school children from Berhampur, Odisha & tried to find out if any relationship is there or not between obesity & hypertension in those study population. 
Methods: In a cross sectional study we select the children from different schools of Berhampur, Odisha during 2009-11. In this, 5155 student of 10-16 Yrs. of age group were taken. Blood pressure & anthropometrics data were collected. Overweight & obesity were defined by body mass index for gender & age. Gender, age & height were considered for determining hypertension. The hypertensive children were followed up at 6 monthly intervals, 3 times, to look for persistent hypertension. 
Results: out of 5155 students, 10.4% students were over weight & 3.6% were obese (3.68%). 190(3.68%) students were found to have sustained hypertension. The number of girls students having hypertension (4.47%) was more then boys (3.2%). There was a significant association of hypertension in obese group in comparison to overweight & normal blood pressure group. 
Conclusion: Prevalence of sustained hypertension overweight & obesity is on rise in adolescent age groups in this part of world. Possible related factors for this current trend may be the increasing sedentary life style, altered eating habits, and increased fat content of diet. The result suggests the need for more public awareness & prevention programs for childhood obesity & hypertension.

For easy and clear understanding of biomechanics, the inter-relationships of simple machines and musculoskeletal functions have always been attempted and established. The masticatory biomechanics is still under research because there is wide range of findings on masticatory muscles including the lack of complete clarity on their line of actions and lever arms too. As a prospective contribution to advanced research on mastication and management of Temporomandibular joint, there are two innovative concepts discussed in this article; (1) Strong correlation between stapling mechanics and Temporomandibular joint biomechanics at molar region (2) Scope for a new unidentified lever system named as fourth class lever. The principal intent of this article is to share these two innovative concepts to be explored further for practical applications in the field of Physical therapy, Dentistry and Engineering.

ABSTRACT:
Exploration of the biomechanical interaction of musculoskeletal system keeps endorsing the fact that the human body can ensure efficient biomechanics without any kinematic aberrations in the joints. Excluding the injuries caused by unexpected collisions, the major potential factors that ruin the protective configuration of musculoskeletal system can be lack of exercise and incorrect exercise. A fresh perspective; ‘force couple mechanics (FCM) on femur’ (torque on femur by two muscular forces in opposite directions at two different locations), has been discussed in this article using fundamental information on anatomical linkages of muscles & correlation of various scientific reports, finally expected to stimulate electromyographic studies to dig out further scientific data.

ABSTRACT:
Arthrofibrosis following total knee arthroplasty is an uncommon complication defined as less than 80 degrees of knee flexion 6-8 weeks post operatively. It is characterized by abnormal scarring of the joint in which the formation of dense fibrous tissue and tissue metaplasia prevent normal range of motion. Clinical features include limited knee Range of motion with extension deficit, pain with activities of daily living and unusual amount of pain and swelling post operatively in the absence of infection, bleeding or mechanical complications. We present case of 55 years old female who undergone for total knee replacement before 3 months and presented to our department with complain of knee pain and swelling with activities of daily living. She was diagnosed on the basis of clinical examination. Her detailed evaluation was carried out and Physiotherapy treatment was started.

ABSTRACT:
Background: Physical activity and exercise following a spinal cord injury is a central part of the rehabilitation programme for everyone with a spinal cord injury. However, the level and extent of the injury as well as how much time has passed since the injury is of even greater importance. Patients should be mobilised as soon as medically possible after a spinal cord injury.
In the present study, during the active rehabilitation phase, emphasises of treatment was on maximising functional independence (transfers, self-care including- bathing, toileting, dressing); and later on emphasis was on general fitness, co-ordination and balance.
Outcomes: Level of functional independence was improved i.e. the patient was able to transfer from bed to chair; he was able to stand and walk with the help of walker. He was able to do his self-care, i.e. bathing, toileting, dressing, eating, which was not possible earlier.

ABSTRACT:
Introduction: The change in arterial blood pressure and heart rate is one of the generally known physiological changes in aging of man. The present study was intended to evaluate BP and HR changes with age.
Aim The objective of the study is to know the prevalence of orthostatic hypotension in healthy geriatric subjects.
Materials and Methods: The subjects selected are 120 in number with age group 35 to >65 years of both sex and they are categorized into 4 groups based on their age. Group- I: The age of 35-45 years; Group-II:  The age of 45-55 years; Group-III: The age of 55-65 Years; Group-IV: The age of >65 years. Blood Pressure is recorded by using manual sphygmomanometer and orthostatic test was conducted in all the subjects. Heart Rate was calculated by using R-R interval by ECG machine. 
Results: The examination of parameters, body mass index, Heart Rate, systolic and diastolic Blood Pressure during resting, immediate standing, after three minutes of standing i.e., postural variations were estimated for all subjects. The data was analyzed by using descriptive and inferential statistics.
Discussion: The findings suggested that the mean SBP and DBP are higher in Group IV compared to Group I which is statistically highly significant. After immediate standing in all the subjects the SBP is decreased and DBP is increased and this variation is variable in different groups. The Heart Rate in all four Groups increases. And this variation is also variable in different groups. 
Conclusion: This study concluded that BP increases with increase in age due to stiffening of Blood vessels but the postural decrease in SBP in standing from lying down posture was more in elderly. Orthostatic Hypotension was found in only one elderly. So Orthostatic Hypotension was less prevalent in healthy elderly.

ABSTRACT:
Study Design: Methodological Study, translation
Objective: To translate the original English version of Oswestry Disability Index Version-2.1a into Hindi language and to assess its content validity, Internal Consistency and Test-Retest reliability.
Summary of Background Data: ODI has been validated, and used as an outcome measure for various Low Back Pain conditions and its Hindi version is not available.
Methods: The translation was carried out in accordance with the Linguistic validation of a Patient Reported Outcomes Measure, provided by the Mapi institute. Content validity was evaluated through Qualitative (Wallace & McKenzie) and Quantitative methods (Lawshe). Internal Consistency and Test-retest Reliability was examined on 50 patients.
Results: Internal consistency for Hindi ODI version 2.1a was excellent with Cronbach‟s alpha=0.947. The intraclass correlation coefficient of test–retest reliability was 0.900.
Conclusion: The results of this study indicate that the Hindi version of the ODI 2.1a is a reliable and valid instrument for the measurement of disability in patients with low back pain.
