      <title>NEUROPHYSIOLOGICAL CHANGES IN CONTEXT TO DURATION OF EXPOSURE IN INSULIN DEPENDENT AND NON INSULIN DEPENDENT DIABETES MELLITUS</title>

ABSTRACT
Background: Diabetes mellitus is characterized by inability of the body to metabolize glucose properly. The prevalence of diabetes is rapidly rising all over the globe at an alarming rate. IDDM and NIDDM shows effect on central nervous system, peripheral nervous system, autonomic nervous system, cardiovascular system and other physiological system. A common complication due to the IDDM and NIDDM includes peripheral neuropathy, retinopathy, nephropathy and vascular complication. Insulin and non-insulin dependent diabetes mellitus - both affect the peripheral nervous system depending on the duration of exposure and blood glucose level. 
Materials and Methods: 120 healthy individuals with IDDM and NIDDM were included (screened with SF-36), with age limit between 25 to 60 years. Those individuals having a history of hospitalization in last 1 year, acute fever, present history of radiculopathy and open wound were excluded. Nerve conduction studies of common peroneal, tibial and sural nerves were examined in both groups. Distal Latency, NCV and CMAP/SNAP were taken as outcome measures. 
Result and Discussion: Bio-statistical analysis has been done using Mann-Whitney test. Result suggest that long duration of exposure has significant (p<0.05) effect on Neurophysiological changes compare to short duration of exposure in IDDM and NIDDM. With longer exposure of diabetes, there is increase in latency, decrease in amplitude and NCV. 
Conclusion: Long duration of exposure shows significant Neurophysiological changes compare to short duration of exposure in IDDM and NIDDM.

ABSTRACT

Background: Much research is done on the association between the hip internal rotation and low back pain, but there is no retrievable clinical trial done on the treatment of the hip internal rotation deficit and its effect on low back pain making it necessary to establish the hypothesis by a randomized clinical trial.
Material and Methods: 52 subjects who met the inclusion criteria were randomized into either control group or experimental group. Control group was given conventional treatment alone which included moist heat and TENS whereas Experimental group was given hip internal rotation mobilization and the same conventional treatment for six consecutive days. Outcome measures were Visual Analogue Scale, Oswestry Disability Index and Internal Rotation Range of Motion were recorded.
Results: The results showed no statistically significant change between the experimental and control group except in Internal Rotation Range which improved significantly in experimental group than control group(P < 0.001), but not in Visual Analogue Scale(P= 0.564) and  Oswestry Disability Index(P = 0.516).
Conclusion:  Experimental group and control group showed significant improvements in both pain and disability.

ABSTRACT

Background and purpose: Children with hemophilia often have intra-articular and intra-muscular bleeding which have a worse impact on postural stability and balance.  Therefore, the purpose of this study was to investigate the effect of sensorimotor training on balance and perceived pain in children with hemophilia. 
Subjects and Methods: Thirty hemophilic boys mean age (12.06±1.2) were divided randomly into two equal groups. Control group (n=15) received conventional physical therapy program and study group (n=15) received the same program in addition to sensorimotor training exercises. Baseline assessment included stability indices and perceived pain, was conducted at the beginning of the study and after 8 weeks of treatment program. 
Results: There was no significant difference between both groups in the pre-treatment mean values of all measured variables. Significant improvement was observed in the two groups between pre and post treatment measured outcomes. Furthermore, the study group recorded significantly better improvement in balance and pain parameters compared with the control group. 
Conclusion: Sensorimotor training could be an excellent supplement to regularly rehabilitation program used for children with hemophilia. It produces positive effects on balance and perceived pain parameters for these patients.

ABSTRACT

Background: Obesity and osteoporosis poses major health problems in India. Both have multifactorial etiologies including genetic and environmental factors. Osteopenia seems to start at an early age of 20 years in Indian female Population and after 35 years there is a sharp decline in bone mineral density with peak at old age. Recent studies show that low bone mineral density (BMD) and high body mass index (BMI) impairs physical function and may cause morbidity and mortality in older adults. However, there is a need to evaluate association of BMI and BMD with balance and mobility in young adults, so that risk factors can be modified and effective treatment strategies to improve BMD and BMI can be incorporated in young age. 
Materials and Methods: Analytical correlational study includes 450 female participants in the age group of 20-40 years. BMI is calculated using height and weight measurements. BMD is measured by Ultrasound bone densitometer. For tests of balance and mobility, narrow walk test, obstacle walk test, figure-of-8 walk test, and gait speed is used.
Hypothesis: Risk factors such as dietary habits, sedentary lifestyle, decreased sun exposure, hormonal imbalance due to stress etc can result in decrease in BMD and increase in BMI in young female adults. Also, biomechanical and neurophysiological changes that occurs in osteoporosis and obesity can affect balance and mobility. We hypothesized that, decrease in BMD and increase in BMI can affect or impair balance in young female adults. 	
Clinical Importance: This study will help to create awareness socially in younger females about risk factors of osteopenia, osteoporosis and obesity and its detrimental effects on functional mobility in females. Therapeutic exercises and changes in diet and lifestyle can be incorporated in young age accordingly that will help them to achieve optimum healthy bone mass and body weight improving their health and quality of life in future.

ABSTRACT

Piriformis syndrome is a clinical condition of non-discogenic sciatica caused by compression of the sciatic nerve by the piriformis muscle and its patho-physiology is not fully understood. It has variable etiologies. The major etiology known includes spasm, edema and inflammation of the piriformis muscle, and sciatic nerve compression by the muscle later on. Patients can be diagnosed immediately with a comprehensive clinical examination and early diagnosis which makes the treatment much easier. A case of piriformis syndrome diagnosed in a patient who presented with radicular low back pain is reported in this study. Differential diagnosis of the piriformis syndrome, a very rare cause of low back pain, should be thoroughly made with the exclusion of other conditions.

ABSTRACT

Background: Mechanical neck pain (MNP) is a common neck disorders affecting middle-aged population. Magnetotherapy is considered as a safe and non-invasive physical therapy modality used in the treatment of musculoskeletal pain.
Objective: The purpose of this study was to investigate the effect of pulsed electromagnetic field (PEMF) on pain and functional improvement in chronic MNP.
Subjects and Methods: A total of 60 male patients participated in this study. Their mean age, weight, height, body mass index (BMI) and duration of illness were 23.03 (2.239) years, 63.43 (5.195) Kg, 1.662 (5.1092) cm, 22.98 (1.879) Kg/m2, 4.483(1.228) months respectively. They were randomly assigned in to two groups. Group I (30 Patients) was treated with PEMF plus exercises (PEMF+EX) group and group II (30 Patients) treated with placebo magnetic plus exercises (PL+EX) group. Exercise program included active range of motion, muscle stretching and strengthening exercises applied two sessions/week for 6 weeks. PEMF was applied with 20 Hz, 0.8 mT for 20 minutes two sessions/week for 6 weeks. Pain level was measured by visual analog scale (VAS) and neck functions were measured by neck disability index (NDI). The level of significance was set at < 0.05.
Results: Wilcoxon matched-pairs signed-ranks test revealed significant differences in VAS and NDI in PEMF+EX and PL+EX groups (p <0.0001). Mann-Whitney (MW) Test showed significant decreased in VAS and NDI scores both treatment (p <0.0001) with more significant decrease in PEMF group than PL+EX group.
Conclusion: PEMF combined with exercise was effective more than exercises alone in decreasing the scores of VAS and NDI in Chronic MNP.

ABSTRACT

Background: Skeletal muscle fatigue is one of the most common problems encountered in general practice clinic population and sport activities. Interferential currents therapy is widely used by physiotherapists throughout the world to manage a range of musculoskeletal condition. 
Objective: To find out the effect of pre-exercise sweep frequency interferential currents therapy on the induced fatigue of biceps brachia muscle in normal healthy untrained male subjects.
Material and Methods: Twenty healthy male subjects were participated and randomly allocated to receive two treatment sessions of interferential currents therapy before inducing biceps brachia muscle fatigue with 7 days apart. Active interferential currents session with trapezoid sweep frequency that set between a low beat frequency of 5 Hz & a high beat frequency of 110 Hz for thirty minutes & Placebo interferential currents session. After the treatment session each subject was asked to perform voluntary elbow flexion repetitions with 30% of their one repetition maximum using dumbbells until they reached fatigue determined by inability to reach 90 degrees of elbow flexion. Pain intensity level before and after reaching fatigue was measured by visual analogue scale, number of concentric contraction before fatigue and time elapsed to reaching fatigue point were measures for all subjects in the two sessions. 
Results: A significant difference (p≤0.05) was found between Active and  Placebo interferential currents sessions, when the means ± SD of the two sessions were compared in a terms of pain intensity level , number of submaximal repetitions & time elapsed to reach biceps brachia muscle fatigue in favour of active interferential currents therapy.
Conclusion: The application of sweep frequency interferential current with the used parameter could be effective in delaying the development of skeletal muscle fatigue and enhancing skeletal muscle performance.

ABSTRACT

Introduction: Frozen shoulder is a common disease which causes significant morbidity. Frozen shoulder is a common shoulder disorder characterized by pain and gradual loss of active and passive glenohumeral motion that occurs in 2–5% of the general population. However, the etiology and pathophysiologic mechanisms that lead to the development of frozen shoulder are poorly understood, and there is no consensus regarding optimal treatment.
Materials and Methods: Total number of 850 patients were initially recruited during a one year period between January 2014andJanuary 2015;we treated 70 patients with a diagnosis of primary frozen shoulder resistant to conservative treatment by manipulation under anaesthetic and arthroscopic release of the rotator interval, at a mean time from onset of 5 months. 
Result and Discussion: Biopsies were taken from this site and histological and immunocytochemical analysis was performed to identify thetypes of cell present. The tissue was characterised by the presence of fibroblasts,proliferating fibroblasts and chronic inflammatory cells. The infiltrate of chronic inflammatory cells was predominantly made up of mast cells, with T cells, B cells and macrophages also present.
Conclusion: In conclusion, arthroscope biopsy material from the rotator interval in patients with frozen shoulder revealed immunocytochemical evidence of both chronic inflammation and proliferative fibrosis, supporting the theory that this is both a chronic inflammatory and a fibrotic condition.Together with the presence of high vascularity and nerve tissue, this inflammation and fibrosis explains why frozen shoulder is such a painful and stiff condition.

ABSTRACT

Background: A number of complications of pregnancy that might occur during the period that women are still working can be regarded as adverse maternal outcome. Since studies on occupational risk factors during pregnancy showed conflicting results, it is unclear how pregnant women should manage. So, the present study aims to find out such occupational risk factors and energy expenditure of working pregnant women in Aurangabad city, Maharashtra, India. 
Materials and Methods: 120 working pregnant women in their second trimester who were visiting the antenatal clinic at Mgm medical college and hospital, Aurangabad were selected. They were interviewed using demographic data and pregnancy physical activity questionnaire [PPAQ]. Sample was chosen according to inclusive and exclusive criteria and scoring was done which divides the physical activity into sedentary, light, moderate, vigorous, household, occupational and sports. 
Results: It was however observed that the higher amount of energy 18.58% was expended on occupational activities. Maximum participants in this study 46.66% were sedentary worker Energy expenditure for occupational activities was highest 28.01% for other heavy type of work and lowest for the sedentary work 16.64%. 
Conclusion: The study recommended that most of the pregnant working women were physically active in their second trimester of pregnancy and most of their energy was expended on occupational activities. The present study also concludes that risk factors of working pregnant ladies are less as the percentage of sedentary type of workers in this study are more than heavy type of workers.

ABSTRACT

Background: Chronic obstructive pulmonary disease (COPD) is characterized by acute exacerbation causing breathlessness, cough and reduced chest expansion. COPD is a currently the fourth leading cause of death in the world. In COPD, expiratory airflow is limited due to obstruction, leading to air trapping and hyperinflation.
Materials and Methods: It was an experimental study carried out at the pulmonology department of Krishna hospital, Karad. A total of 40 subjects were selected by convenient sampling and they were randomly allocated into two groups .Group A received conventional chest physiotherapy and Group B received shoulder and thoracic mobility exercises along with conventional chest physiotherapy. Treatment duration was 30 minutes/session, twice daily for three days. The outcome measures were chest expansion using inch tape and Modified Borg Scale (MBS) for dyspnoea.
Result and Discussion: Paired t test and wilcoxon matched pairs test was used to analyze the effect within the groups. There was significant difference for chest expansion at 3 levels in group A (p=0.0286), (p=0.0358), (p =0.0298) and extremely significant difference in group B (p<0.0001), (p<0.0001), (p<0.0001). In MBS, there was extremely significant difference (p<0.0001) in group A also there was extremely significant difference (p<0.0001) in group B.  
Unpaired t test and Mann Whitney test was used to analyse the effect between the groups. There was extremely significant difference for chest expansion in post values (p<0.0001), (p<0.0001), (p<0.0001) between group A and B. In MBS, post value (p<0.0001) was extremely significant between group A and B.
Conclusion: Hence, It was concluded that shoulder and thoracic mobility exercises were more effective in improving chest expansion and reducing dyspnoea in COPD patients.

ABSTRACT

Introduction: Low back pain is a very common health problem associated with considerable disability and costs to society. Low back pain is becoming increasingly common. Approximately 70-90% of our population suffers from low back pain or has suffered from multiple episodes of low back pain. Pain aggravated by extension, side-flexion, rotation, standing, walking, sitting, and exercise in general, and relieved by changing in position. Many different therapeutic interventions are used in the management of low back pain. Swiss ball exercises have become an important part of the fitness regimen and widely used exercises for low back ache.     
Materials and Methods: The study was of an experimental design, with 30 subjects, 13 were female, 17 were male, and all subjects were assigned according to criteria (inclusion & exclusion) and carried out at physiotherapy OPD of CSS Hospital, Meerut. Disability & pain were assessed by using the ODI & VAS score respectively while endurance of abdominal and trunk extensors muscles assessed by using DAET and DEET score. The subjects were reassessed after completion of 6 weeks of intervention. The collected data were of mean and standard deviation of ODI, VAS, DAET and DEET score and has been analyzed using SPSS software. The study was done to find out the efficacy of Swiss ball exercises in reducing pain, disability and improving endurance of muscles (abdominal and trunk) in patients with mechanical low back ache. 
Results: The results showed that there were significant difference in pain, disability and muscle endurance with their VAS, ODI score (p=0.001) and DAET, DEET score respectively.
Conclusion:  Study concluded that the difference from 1st to 42nd day in VAS, ODI, DEET & DEET score which shows that swiss ball exercises are effective to decrease  pain, disability and improve muscle endurance.

      <title>PHYSIOTHERAPY AWARENESS IN MEDICAL AND NON MEDICAL POPULATION: A SOCIAL MEDIA SURVEY </title>

ABSTRACT

Background: Physiotherapy is one of the oldest branches of healthcare profession which is concerned with holistically rehabilitating the  patient. In recent years, the field has evolved rapidly providing evidence based quality health services. Inspite of people awailing physiotherapy services its awareness to a certain extent appears to be less. The purpose of this study was to find level of awareness of Physiotherapy among medical professionals and general population.
Materials and Methods: A semi structured, self-administered questionnaire was prepared on Google Forms with open and close ended questionnaire. They were circulated among medical and general population via social media, accessible to all for a period of one week. The data was calculated automatically by Google spreadsheet.
Results: 103 people from medical and 153 people from general population filled the questionnaire.90% of medical professional and 78% of general population knew physiotherapy field. Both groups were aware about specialties of Physiotherapy. However awareness was less in specialty such as Cardiovascular and pulmonary conditions, Plastic surgery, Oncology and Post-surgery physiotherapy in both the populations.
Conclusion: There is a dire need to take measures to improve awareness of the field and its specialties. Education program should be done on large scale to improve the knowledge and perception of Physiotherapy among medical and general population

      <title>ISOKINETIC STRENGTH TRAINING IN PATIENTS WITH STROKE: EFFECTS ON MUSCLE STRENGTH, GAIT AND FUNCTIONAL MOBILITY</title>

ABSTRACT

Background: Muscle weakness in the lower limbs has adverse effects on walking performance and functional mobility in patients with stroke. Therefore, interventions aiming to improve muscle strength post stroke are considered crucial part of stroke rehabilitation.
Objective: To determine the effectiveness of isokinetic strength training for knee and ankle muscles of the affected side on muscle strength, gait and functional mobility in chronic stroke.
Materials and Methods: Thirty patients with stroke of both sexes (46-62 years) shared in this study. They were randomly assigned into two equal groups; experimental and control groups. Patients in the experimental group received a traditional physical therapy program in addition to isokinetic training for knee and ankle muscles in the affected side. Those in the control group received only the traditional physical therapy. Treatment was provided three times per week for eight successive weeks. The participants received pre and post-treatment assessments for the peak torque of the trained muscles, gait parameters and functional mobility as measured by the Timed Up and Go (TUG) test.
Results: Patients in both groups showed significant improvement post treatment in peak torque of knee and ankle muscles, gait parameters and TUG test (p<0.05). Additionally, after the intervention, between-group comparison revealed significant difference for peak torque of both knee and ankle muscles (p<0.05), walk speed (t=2.44, p=0.02), gait cycle time (t=-3.43, p=0.002), single limb support (t=3.58, p=0.001) and TUG test (t=-3.45, p=0.002) in favor of the experimental group.
Conclusion: Isokinetic strength training of the affected lower limb muscles in conjunction with proper physical therapy exercise program is effective in improving muscle strength, walking performance and functional mobility in patients with stroke.

      <title>EFFICACY OF ONE TIME TRANSCUTANEOUS ELECTRICAL NEUROMUSCULAR STIMULATION (TENS) IN NEXT TWO MENSTRUAL CYCLES IN PRIMARY DYSMENORRHEA</title>

ABSTRACT

Background: Dysmenorrhea refers to the occurrence of painful menstrual cramps of uterine origin. TENS may be an alternative treatment option for women with dysmenorrhea who wish to stop using non-steroidal anti-inflammatory drugs (NSAIDs), oral contraceptives, or other analgesics because the existing medication is ineffective, has unacceptable adverse effects, or due to personal choice. An effective non-pharmacological method of treating dysmenorrhea would be of great potential value in treating dysmenorrhea.
Materials and Methods: 50 females with age group of 20-30 years having moderate or severe degree of disability due to dysmenorrhea with VAS score ≥ 7 without athletic background were taken into the study. Conventional TENS was applied on 1st and 2nd day of menses over the abdomen in criss-cross pattern and effect of TENS was evaluated in present and next two menstrual cycles. Assessment tools were Visual analogue scale (VAS), Short-form McGill Pain Questionnaire(SF-MPQ) and Short-form Moos Menstrual Distress Questionnaire (SF-MMDQ) and Outcome measures were Pain ( assessed by VAS and SF-MPQ), Quality of life (assessed by SF-MMDQ) and Number of analgesics used.
Results: The present study demonstrates that there is decrease in pain and improvement in quality of life after application of TENS in present cycle as well as  in next two menstrual cycles in  primary dysmenorrhea as evident by decrease in VAS (mean) score, SF-MPQ (mean) score and SF-MMDQ (mean) score(p=0.05). 
Conclusion: Conventional TENS is effective in relieving pain and improving quality of life in moderate degree of disability due to primary dysmenorrhea. Majority of subjects have shown relief of pain and improvement in quality of life in next 2 menstrual cycles and didn’t need any analgesic after one time TENS treatment.

      <title>INFLUENCE OF GLUTEUS MAXIMUS INHIBITION ON UPPER TRAPEZIUS OVERACTIVITY IN CHRONIC MECHANICAL NECK PAIN WITH RADICULOPATHY</title>

ABSTRACT

Background: Mechanical neck pain is the most common type of neck pain and commonly to accompany with radiculopathy. Patients of neck pain exhibit greater activation of accessory muscles, (sternocledomastoid, anterior scalene, and upper trapezius muscles) and may also show changed patterns of motor control of other posture muscles as pelvic muscles for reducing activation of painful muscles of neck. 
Aim of the study: To determine if there is an association between gluteus maximus inhibition and over activity of upper fibers of trapezius in patients with chronic mechanical neck pain with radiculopathy. 
Materials and Methods: Forty female patients participated in this study diagnosed as chronic mechanical neck pain with radiculopathy. Amplitude and onset of muscle activation were assessed by using the surface electromyograghy (EMG) during prone hip extension test. 
Results: The results of this study demonstrated that there is no correlation between the amplitude of  EMG activity of right and left gluteus maximus and the amplitude of EMG activity of  right and left upper trapezius (P<0.05). 
Conclusion: It can be concluded that the overactivity of the upper trapezius muscle in patients with chronic mechanical neck pain with radiculopathy is not related to the inhibition of the gluteus maximus muscle during prone hip extension test.
